Make sure you have a full discharge summary, a list of all medications with dosage instructions, your follow-up appointment dates, and emergency contact numbers for your medical team. Confirm who will collect you and ensure your home is ready.
Before you arrive home, remove trip hazards (rugs, cords), ensure frequently used items are within easy reach, set up a comfortable rest area on the ground floor if stairs are a challenge, and install grab rails in the bathroom if needed.
Use a pill organiser or phone reminders to stay on top of your medication schedule. Keep a written log of what you've taken and when. Never adjust dosages without consulting your doctor, and keep emergency contacts handy.
Recovery takes energy. Prioritise sleep, eat nutritious meals even when appetite is low, and stay hydrated. If preparing food feels too difficult, in-home support with meal preparation is something WSRC can help with.
